QUALIFICATIONS:
Junior Prep
The applicant must already be four (4) years and six (6) months old by June of next school year.
Senior Prep
The applicant must already be five (5) years and six (6) months old, by June of next school year.
Grades 1 to 7 and 1st and 2nd Year
High School
The applicant must have a general average of at least 85% with no
grade lower than 80% in any subject and in conduct during any grading
period.
1st and 2nd Year High school
The applicant should have completed Grade 7. In cases where Grade 7
is not offered in the previous school, the applicant will be advised
to apply to a lower level. High School applicants must have taken all subjects required by DLSZ in the lower levels.
Applicant must not be less than 12 years old but not more than
14 years old by June of the following school year.
Former DLSZ students
who left without conditions and have been out for at least 2 years are
classified as transferees when they come back and must undergo the usual
procedure for application. Such students will rejoin the level of their
former classmates.
REQUIREMENTS:
- For Filipino Students
- Form 138 or Report Card of the previous academic year, and a photocopy of the Report Card of the current school year
with at least the 1st quarter or 1st term grades;
For Grade One applicants: In the absence of the confirmation
for promotion to Grade One, a certificate that the pupil
will be promoted to Grade One after completion of his/her
present grade level must also be submitted;
- Certified True Copy of Birth Certificate issued by the National Statistics Office;
- Certified True Copy of Baptismal Certificate (for Catholics);
- For Grades 2 to 7 and 1st and 2nd Year applicants: A
Certificate of Good Moral Character from the previous school
attended;
- For 1st Year applicants: Certified True Copy of the
Student's Permanent Record (Form 137) marked for evaluation
purposes only;
- Certification from AAVA, if applicant is a resident of Ayala Alabang Village;
- Document to show if parent is an alumnus/alumna of ANY La Salle school;
- Recommendation form, which is available at the
Admissions Office or may be downloaded here:
- Two (2) recent 1x1 photos with complete name of the
student to be printed at the back of each picture (close up,
white background)
For Boys: proper haircut, no colored hair, no long bangs, no
earrings
For Girls: no dangling earrings, no make-up, simple hairdo
- For Foreign Students (in addition to the above-mentioned documents)
- Copy of Birth Certificate from the Office of the Civil Registry
of the respective country of birth, translated into English, or Embassy
Certification of birth details;
- Special Study Permit issued by the Bureau of Immigration upon receipt of the proof of enrollment from the school;
- Identification Certificate as Filipino Citizen issued by the Bureau of Immigration (for applicants with dual citizenship); and
- Final Report Card from Grade 1 up to the most recent level of
schooling. A description of the different subjects must be included.
PROCEDURES:
Walk-In Applicants:
- Secure an Application Form from the Admissions Office after considering the qualifications for admission.
- Fill out the Application Form completely and attach all required documents.
- Pay the Application Fee of PhP 550.00 for Filipinos or USD 75.00 for
foreign students at the Accounting Office. The fee is non-refundable.
- Submit the Application Form, application fee receipt, and all requirements to the Admissions Office.
- Test Permit will be given after complying with the above requirements and procedures.
On-Line Applicants:
- Fill out the Application Form
here and then click Submit. Print out Confirmation Slip.
- Submit Confirmation Slip to the Admissions Office, together with the required documents.
- Pay the Application Fee at the Accounting Office.
- Present the Application Fee receipt to the Admissions personnel to secure the test permit.
ONLINE APPLICATION IS HIGHLY ENCOURAGED. ONLY APPLICANTS WHO HAVE SUBMITTED COMPLETE REQUIREMENTS WILL BE GIVEN A TEST PERMIT.
RELEASE OF TEST RESULTS
The Admissions Office prepares and releases the test results in a
sealed envelope two (2) weeks after the testing date. These may be
claimed from the Admissions/Registrar's Office (Monday to Friday,
8:00 am - 4:00 pm only) or may also be checked
online.
- Only parents of applicants are eligible to receive the results of
their child's tests. Other representatives will have to present an
authorization letter to claim the results.
- Telephone inquiries regarding the test results will NOT be entertained.
- Parents of accepted applicants should confirm enrollment on the dates indicated in the accepted letter

- Pay the Development Fee at the Accounting Office
This is a non-refundable payment made upon confirmation. (Only Filipino Students, however, are given an option to pay in two years at PhP 15,000.00 per enrollment.)
- PhP 30,000.00 - Filipinos
- USD 1,500.00 - Foreign Students
- The Development Fee is included in the tuition fee for SPIS Students
- Submit the accomplished forms, official receipt of Development Fee payment
and original copy of your child's Report Card to the Registrar's Office.

THE GIVEN TIME FRAME MAY FORFEIT HIS/HER ADMISSION

NOTES
- For Prep applicants, enrollment will start on February 8,
2011. For other levels, enrollment is scheduled on March 8
to May 3, 2011 (for both old and new students).
